Your Sacramental Records
- If you are a baptized Catholic you will need to request a new copy of your baptismal certificate from the parish of your baptism, noting all Sacraments received. Please ask the Parish of your baptism to send your certificate direct to our Parish Office.
- Non-Catholic Christians must provide evidence of baptism (e.g, a certificate/letter from the church’s secretary, or a signed affidavit by a witness to the baptism). These are sent direct to the Parish Office.
If any of your Sacraments were received at St. Agnes or St. Athanasius, Reading, please follow this link for us to draw up the appropriate Sacramental Certificate(s).
